Financial Assistance/Charity Care
Kewanee Hospital accepts all patients, regardless of their ability to pay. We continue our commitment to the communities we serve. Our mission, to improve the health and care of those we serve, is ever in the forefront as we look for new ways to benefit our community and ensure local access to quality healthcare services.
For determining financial assistance or charity care eligibility at Kewanee Hospital, please request and complete a Financial Assistance Statement, attach the following financial records, and submit to the Business Office:

Complete tax return for previous calendar year including W-2 forms. Students must provide their parents’ income tax information for the previous calendar year.

Paycheck stubs for a period of 3 consecutive months, prior to month application received.

A letter from the financial and/or lending institution reflecting the current value and loan balance of assets.

Checking/savings account statement for the past 3 months. If there is no checking/savings account, a notarized letter from the bank stating this fact is needed.

Statement of monthly benefits from Social Security, pension, unemployment, if applicable.
